Overview
We have an exciting opportunity for a District Nurse Clinical Lead to join our caring Tamar Valley and Launceston Community nursing teams in the area of East Cornwall.
Main duties of the job
* Monitor the quality of care provision and identify and lead on areas of service development.
* Provide clinical leadership for all patients within the community nursing caseload, working in an integrated team and across professional and organisational boundaries.
* Diagnose, prescribe, plan and evaluate care to ensure provision of high standards of care to patients and their families in the community.
* Work with other health and social care agencies and professionals to support care delivery and liaison.

Person Specification: Education and Qualifications
* Registered Nurse
* Relevant post registration qualification - specialist practitioner qualification (District Nursing) or working towards
* Independent non-medical prescribing qualification or equivalent or working towards
* MSc Nurse Practitioner qualification or equivalent
Experience and Skills
* Previous relevant experience of working within a community nursing team
* Highly developed clinical expertise and knowledge in all aspects of community nursing care
* Highly specialist assessment, planning, treatment and evaluation skills
* Excellent written and oral communication skills including ability to deal with challenging and sensitive situations
Desirable criteria
* Experience of leadership and leading change or project implementation
Additional information
Contract: Permanent, Part time – 30 hours per week. Band 7. Site: Callington Health Centre and Launceston Hospital. Salary: £47,810 – £54,710 Pro Rata. Closing date: 28/10/2025 23:59.
