Lead Mental Health Support Worker
Location: Farnborough, North Camp
Type: Permanent, part-time (30 hours per week), working on a rota pattern
Pay: £13.21 per hour (£25,838 per annum, pro rata) with benefits including Health Cash Plan

Typical day as a Mental Health Lead Support Worker (Support Coordinator):
* Support working-age adults with mental health needs, substance misuse, physical disabilities, learning disabilities, and autism.
* Start your shift with a handover to get updates on customers and any changes in their presentation or risk.
* Manage your own diary and support customers with activities like cooking, shopping, budgeting, maintaining tenancies, and moving to independent living.
* Conduct risk assessments, support planning, goal setting, and reviews.
* Engage in casual chats and collaborate with colleagues and external agencies.

You bring:
* Passion for supporting customers to live their best lives with attention to detail.
* Experience in creating person-centered support plans and supporting colleagues with them.
* Experience in coordinating and assessing customer referrals.
* Ability to work independently, stay calm under pressure, and be resilient.
