Overview
We welcome applications from both experienced staff nurses and newly qualified nurses who are keen to start a career in Public Health Nursing. You will join the service as we progress our 0-19 pathways, working more closely with Health Visiting services and our partners in education, social care and Early Help to take a family approach to the promotion of health and wellbeing, prevention and early intervention. Post holders will work within a team under the direction of a Specialist Practitioner School Nurse to deliver the Healthy Child Programme 5-19 in schools and other education settings. You will develop health promotion activities, work with partners in education, social care, Early Help and the voluntary sector to deliver services to local communities. As a staff nurse you will hold a caseload of children requiring targeted and specialist HCP level support. You will carry out holistic health assessments and plan care for children and young people.
Responsibilities
* Work as part of team to provide advice, care and support to children and young people, their parents/carers in a locality of the borough
* Assess, plan, implement and evaluate care for individual children and young people
* Contribute to the planning and delivery of health promotion programmes and interventions
* Work as part of multi-disciplinary and agency teams to support children, young people and families
* Undertake health assessments to assess, plan and implement and review care
* Deliver health promotion provision
* Support children and young people in need of safeguarding or in need
* Act as a lead professional for children and young people needing Early Help
* Provide advice and guidance to parents/carers and professionals on a range of health and health related issues
* Support children and young people with continence and enuresis problems
* Be able to produce comprehensive reports and undertake analysis of risk
* Work with professionals from partnership agencies to provide localised support to children and young people
