The PCN Health andWellbeing Coach works alongside a team within general practice and within CLICKPCN and empowers people to take control of their health and wellbeing.

They consider the whole person in addressingexisting issues and encourage the proactive prevention of new illnesses,promoting lifestyle medicine approaches.

Health coachingrevolves around using coaching skills to support people with lower levels ofpatient activation to develop the knowledge, skills and confidence to managetheir health and wellbeing whilst increasing their ability to access andutilise community support offers

The post holder will be an integral part of thegeneral practice team, as well aspart of a wider community groups and their multidisciplinary teams.


Main duties of the job

Manage and prioritise a caseload, in accordance with the health andwellbeing needs of their population through taking an approach that isnon-judgemental, based on strong communication and negotiation skills,while considering the whole person when addressing existing issues.Where required and as appropriate, the Health and Wellbeing Coach willrefer people back to other health professionals within the PCN.

Utilise existing IT and MDT channels to screen patients, with an aim toidentify those that would benefit most from health coaching

Provide personalised support to individuals, their families, and carers tosupport them to be active participants in their own healthcare;empowering them to manage their own health and wellbeing and liveindependently.

Work in partnership with patients to support them to develop their level ofknowledge, skills and confidence enabling them to engage with theirhealth and well-being and subsequently supporting them in shareddecision-making conversations.

Utilise health coaching skills to support to develop the knowledge, skills,and confidence to manage their health and wellbeing, whilst increasingtheir ability to access and utilise community support offers.

Explore and support patient access to a personal health budget, whereappropriate, for their care and support.

CLICK - Chard, Ilminster andLangport - Primary Care Network is located in South Somerset with 5 GPpractices and a diverse population spread across town and rural locations. CLICK has a population size of around 50,000patients.

As a PCN we are forward thinking, innovate and driven todeliver the best patient care for our population. This includes healthpopulation management, and this role ties in with supporting that and trackingthe improvements we can make to patients lives.


Job responsibilities

Coaching and motivating patients through multiple sessions to identifytheir needs, set goals, and supporting patients to achieve theirpersonalised health and care plan objectives.

Providing interventions such as self-management education and peersupport.

Supporting patients to establish and attain goals that are important tothe patient.

Supporting personal choice and positive risk taking while ensuring thatpatients understand the accountability of their own actions anddecisions, thus encouraging the proactive prevention of furtherillnesses.

Working in partnership with the social prescribing service to connectpatients to community-based activities which support them to takeincreased control of their health and wellbeing.

Increasing patient motivation to self-manage and adopt healthybehaviours.

Develop collaborative relationships and work in partnership with health,social care, and community and voluntary sector providers and multidisciplinary teams to holistically support patients wider health and wellbeing, public health, and contributing to the reduction of healthinequalities.

Provide education and specialist expertise to PCN staff, supporting themto improve their skills and understanding of personalised care,behavioural approaches and ensuring consistency in the follow up ofpeoples goals with MDT input.

Raise awareness within the PCN of shared decision-making and decisionsupport tools.


Person Specification


Other requirements

* Has formal individual and group coaching supervision which must come from a suitably qualified or experienced individual
* Working closely in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider, to identify and work alongside people who may need additional support but are not yet ready to benefit fully from social prescribing.
* To deliver the primary key responsibilities outlined in B4.2. of the Network Contract Direct Enhanced Service Contract Specification PCN Requirements.
* Willingness to work outside of core office hours.
* Occupational health clearance.
* Access to own transport and ability to travel.

* High level of written and oral communication skills.
* Understanding of the needs of small volunteer-led community groups and ability to support their development.
* Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance and health and safety.


Experience

* Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
* Experience of working within the VCSE sector (in a paid or unpaid capacity) including with volunteers and small community groups.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations
* Experience of supporting people with their mental health in a paid, unpaid or informal capacity.
* Experience of data collection and providing monitoring information to assess the impact of services.
* Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).


Qualifications

* You are enrolled in, undertaking or have qualified from appropriate health coaching training covering topics outlined in the Health and Wellbeing Coaches webpage.
* You are enrolled in, undertaking or have qualified from appropriate training as set out by the Personalised Care Institute.


Skills

* Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
* Knowledge of community development approaches.
* Clear, polite telephone manner.
* Knowledge of IT systems including ability to use word processing skills, emails and the internet to create simple plans and reports.
* Knowledge of motivational coaching and interview skills.
* Ability to work as a team member and autonomously.
* Additionally, the ability to work under pressure and to meet deadlines.
* Knowledge of VCSE and community services in the locality.
* Knowledge of the personalised care approach.
