Job Summary West Leeds Family Practice is recruiting a fixed term Practice Nurse to cover a period of maternity leave. Our surgery consists of 5 Partners, 10 salaried GPs, 2 ANPs, 3 nurses and 5 HCAs. We are part of the West Leeds Primary Care Network and work closely with our neighbouring practices. We are located across 3 sites, are a teaching practice and host both medical and nursing students. We have 20k patients and are continuously expanding. We are a System 1 practice who are high achievers in QOF, DES, LES and all flu and vaccine services. We are flexible regarding days and times of work. This would suit an experienced practice nurse who does not want to commit to full-time work, but enjoys working as part of a dynamic team.
Main duties
To be fully proficient in chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, as well as other general treatment room duties. It is not expected that the successful applicant has to be proficient in all these areas; however managing long-term conditions is a prerequisite.
About Us
The nursing team is extremely close working and mutually supportive. We have a nurse educator who works both for the practice and the PCN. The successful applicant will be supported in growing their expertise and encouraged to have their say.

Job Description
Responsibilities
* Provide chronic disease management for Diabetes, COPD, Asthma, and CHD/CVD
* Perform cervical screening and promote breast awareness
* Administer adult and paediatric injections
* Provide adult and child immunisations ensuring full achievement of targets
* Ensure safe storage, rotation, and disposal of vaccines and drugs
* Undertake venepuncture and electrocardiography
* Undertake wound care/management/dressing techniques
* Provide advice on lifestyle issues, assisting and supporting patients in changing unhealthy behaviours and refer appropriately
* Ensure that current evidence-based approaches to patient care are standard practice
* Apply infection control measures when handling laboratory specimens, disposing of waste materials, dealing with blood and body fluid spillages and report and treat sharps injuries
* Audit outcomes of care against standards and initiate change, ensuring follow-up audit cycle
* Maintain and update disease registers
Person Specification
Qualifications
* Essential: Qualified Registered Nurse with a current NMC registration
Experience
* Essential: Car driver
* Desirable: Experience of working in GP practice and managing long-term conditions
* Desirable: Experience of working within SystmOne
