About the role
As a Team Leader, you will play a crucial role in supporting the home management team to provide exceptional care at Chertsey Parklands Care Home. Your responsibilities include leading personnel in care planning, evaluating care plans monthly, conducting audits, and implementing action plans. Additionally, you'll aid the transition process for residents being discharged from the home while monitoring their ongoing well‑being. You will manage safe work practices and ensure both personal safety and the safety of those around you. The position requires strong communication skills and the ability to lead in the manager's absence.
Package Details
Shift details: Dementia Unit, shift hours 7:00‑19:30, two weeks rolling rota, one week 3 shifts, one week 4 shifts, working every other weekend.
Responsibilities
* Leading in the person‑centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
* Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
* Leading with monitoring the well‑being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
* Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
Qualifications and Experience
* NVQ Level 3 or equivalent in care for care homes.
* Two years experience working in a senior position with the resident group.
* Ability to communicate effectively both verbally and in writing.
* Demonstrated ability to lead staff and act responsibly in the absence of the manager.

* ADBS Disclosure check, the cost of which will be met by Avery Healthcare.
* Proof of eligibility to work in the UK.
