This is an exciting opportunity for an experienced band 6 nurse to join the Care Home Service, an integrated team within Livewell Southwest on a 12 month secondment / fixed term contract. This role requires specialist nursing expertiseto undertake and be responsible for completion of timely and comprehensiveholistic evidence-based assessments, in a range of settings, and to apply therelevant nationally approved criteria to establish eligibility for services andcare.
The successful applicant will need be response to system pressures & complete a range of tasks, working flexibly & interchanging between hospital and community settings.

Main duties of the job
Undertaketimely, complex & detailed assessments. Prepare reports, plans of care, complexpapers or tools such as the DoH Checklist & Decision Support Tools.
Work in acollaborative manner with HSC colleagues in theco-ordination & best interest of the client.
Work incollaboration with the private sector Care Homes to facilitate staff accessing appropriate education opportunities.
Provide specialist assessmentskills, knowledge & management of patients in the hospital/community whilstensuring the delivery of high calibre care, & efficient use of facilities,manpower & other resources.
Be able toconfidently liaise with all members of MDT & participate in patient case conferences as required.
Liaise with communityteams, & other professionals to promote person centredcare planning appropriate to the complex needs of individuals, ensuring arrangements are made for input/training/follow-up with specialist teams wherenecessary.
Work with teams within LSW, UHP and Devon Integrated Care Board, indeveloping processes. Work within agreedassessment formats with adherence to established pathways.
Work withPCC Adult Regulated Care Team, Care Home Service & other key stakeholders to facilitate the further development & implementationof best practice & communications within the Care Homecommunity.
Work inpartnership with care home & other key stakeholders to escalate & address any concerns raised.

Job responsibilities

Undertake assessments on behalf of care homeproviders in Plymouth according to agreed criteria.
Undertake re-assessments and assist Care HomeManagers by liaising with them where there is an existing resident in hospital,for whom there is a potential change of needs.
Ensure that wherever possible the views and needsof the individual are sought and represented with due regard to the personsmental capacity.
Participatein case conferences as required. Ensure accurate minutes and record of meetingsare kept and distributed as appropriate.
Participatein Best Interest Meetings as necessary.
Maintainaccurate records, which are confidential, up-to-date, and legible and that allcare given has to be documented. These records may be paper or IT based systemand must be maintained as specified in the Organisation Policies, NMCguidelines and Government directives.
Work with clinical teams and care homes to ensurethat discharge documentation, risk assessments, clinical equipment andmedication are in place to accompany the resident on hospital discharge, toensure the best possible outcomes for vulnerable people.
Proactively raise issues and provide solutions inpartnership with other colleagues in relation to change management, workingacross agencies to enhance seamless care and improve patient experience.
Act in a supervisory role, to support, advise andempower Care Home staff to deliver specific elements of care to individualswith complex needs.
Provide data for reporting information as agreed onassessments, information dissemination, discharges, bed-days saved, casestudies.
Work closely with the Bed Bureau Manager toidentify gaps in the market and provide data to support future commissioningintentions and/or projects.
Have good communication skills to effectivelycommunicate with patients, their relatives and carers about sensitive andaccurate information about their condition.
