Care coordinators provide extra time, capacity and expertise tosupport patients in preparing for or in following-up clinical conversationsthey have with primary care professionals. They will work closely with the GPsand other primary care professionals within the surgery to identify and managea caseload of identified patients, making sure that appropriate support is madeavailable to them and their carers, and ensuring that their changing needs areaddressed. They focus delivery of the comprehensive model to reflect localpriorities, health inequalities or population health management riskstratification as required.
It is a role that is officially a Ribblesdale PCN role, but the successful candidate will work at Sabden and Whalley practice only and will be under the HR care of the practice.
Main duties of the job
Proactively identify and work with a cohort of people to support theirpersonalised care requirements, using the available decision support aids.
Work with the data quality lead to contact and invitepatients for annual reviews using the Ardens system.
Help people to manage their needs, answering their queries andsupporting them to make appointments.
Work with patients to ensure they receive vaccinesas required and take part in the annual vaccine campaigns
Ensure that people have good quality information to help them makechoices about their care,
Admin duties as needed including read coding, summarising, insurance reports and typing of referrals,
Work towards localenhanced services under the guidance of the data quality lead.

We are a friendly, supportive team based in Whalley situated in the beautiful Ribble Valley.
5 partners, 6 salaried GPs, ANP, 5 nurses, 2 HCAs, and a large reception and admin team, we are also a long standing training practice for GP training. We also have a housebound nurse team attached to the practice for our care home and housebound patients.
WE also have a PCN team that work with our patients, including a pharmacist, pharmacy techs; and a physio. We also have an onsite dispensary at both sites- to give all of our patients a good quality experience at the surgery.
The team work well together, making the surgery a harmonious place to work.

Experience
* Either hold or be willing to undertake care coordination training as required
* Experience of working in a primary care environment
* Experience of continued professional development
* NHS/primary care general practice experience
