THIS VACANCY IS ONLY OPEN TO CURRENT EMPLOYEES OF NHS AYRSHIRE AND ARRAN
An exciting opportunity has arisen for a Physiotherapist within our Enhanced Intermediate Care Service based at Ayrshire Central Hospital, Irvine.
What you will do:
An exciting opportunity has arisen to become part of the Enhanced Intermediate Care Team to provide short term support for people with conditions that can be safely managed at home or in community but require additional health and social support to facilitate this.
The service can facilitate early and supported discharge from hospital or provide an alternative to hospital admission.
It aims to maximise the long term independence, choice and quality of life and as a consequence reduce the need for on-going support from formal services.
People’s needs are identified using documentation based on a comprehensive multi-factorial care of the elderly assessment. Interdisciplinary treatments, rehabilitation and enablement plans are then created in partnership with the service user.
The focus is on setting goals and achieving outcomes for individuals that enable them to be as independent as possible.
