Community Nurses make a difference every day to the lives of the people they visit at home. We are proud of our Community Nurses who provide increasingly complex care for patients and support for family members to encourage patients to receive the care they need in their own home.
We are looking for newly qualified and registered nurses with a passion for high quality nursing care and experience in acute medicine, emergency care or community nursing. The post holder will provide acute nursing assessment and interventions in patients’ own homes and will ensure that patients are referred onward as appropriate to community services once their care episode is complete.

Responsibilities
As a community nurse you will:
* assess the healthcare needs of patients and families;
* monitor the quality of care they are receiving;
* be professionally accountable for the delivery of care;
* utilise technology to update records as you go.
Patients can be of any age, but you will often care for the elderly, those recently discharged from hospital, terminally ill patients or those with physical disabilities. The service aims to enable patients to remain in their own home environment, working as part of a multi‑disciplinary team to minimise hospital admissions and readmissions.
Must Have Skills
You must be adaptable, resourceful, well‑organised, confident and able to cope in diverse situations. Leadership and management skills are important, as are communication, problem‑solving and good judgement. Driving, access to a vehicle and a reasonable level of physical fitness are required.
Work flexibly, prioritise tasks based on patient need and operate with minimal supervision.

Person Specification
NMC Registered
* NMC registered nurse or student nurse working towards qualification.
* Registered nurse with post‑registration experience within a community setting.
Community Nursing Experience
* Experience in a community nursing setting.
* Experience in venepuncture.
* Palliative care.
* Experience using SystemOne or equivalent patient electronic records systems.
* UK car driver and owner.
