<div><p>We have an exciting opportunity for a Registered Nurse to join our successful Continuing Healthcare (CHC) team in the Wirral. The team provides commissioning services for people with complex care needs who are eligible for NHS‑funded care.</p><h3>Responsibilities</h3><ul><li>Ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded‑Nursing Care, and submitted to the ICB within the service’s key performance indicators.</li><li>Complete all review assessments in line with the service’s key performance indicators.</li><li>Work closely with acute hospitals to support discharge to assessment pathways.</li><li>Accurately input information onto departmental databases and tools used in Continuing Healthcare, adhering to NMC standards for record keeping.</li><li>Assist and support individuals and representatives in relation to the Continuing Healthcare process.</li><li>Commission appropriate packages of care for individuals eligible for CHC or other NHS‑funded care, in line with the Commissioning Policy.</li><li>Case‑manage and review individuals eligible for CHC or other NHS‑funded care in accordance with the National Framework for Continuing Healthcare and NHS‑Funded Nursing Care.</li><li>Provide assurance on the quality of individual packages of care by undertaking reviews as required and reporting quality and safeguarding concerns through the agreed reporting route in a timely manner.</li><li>Undertake urgent reviews of individuals where quality and safety concerns have been identified.</li></ul><p>All new starters will commence in post on a fixed start date each month, in line with our Prepare to Care (P2C) induction programme which offers up to 3 weeks of training to support you to meet the induction and initial training requirements of the role.</p><h3>Qualifications</h3><ul><li>Educated to degree level or equivalent experience in a specialist area.</li><li>Evidence of post‑qualifying and continuing professional development.</li><li>Current NMC registration.</li><li>Full understanding of social services and other local authority functions.</li><li>Conversant with national and local guidance on continuing care/NHS‑funded care for adults.</li><li>Strong understanding of the operational management and clinical role of the NHS.</li><li>Understanding of the patient experience agenda and its contribution to quality.</li><li>Knowledge and understanding of the current NHS agenda and of primary care.</li><li>Understanding of the commissioning agenda, including scope, structure, methods of operation, and inter‑relationships.</li><li>Track record in clinical quality & performance activities.</li><li>Experience with partnership working with other agencies.</li></ul><h3>Additional Requirements</h3><p>Valid driving licence and access to a vehicle for work purposes are essential, as the role ordinarily involves travelling across the Wirral with occasional visits further afield.
