EHCP (Education, Health and Care Plan) Social Care Officer (Fixed Term Contract 12 months)
We are excited to offer a brand new opportunity for two EHCP (Education, Health and Care Plan) Social Care Officers on a full time, 12 month fixed term/secondment basis. (Any successful internal candidate will be offered the position as a secondment and, in line with Section 8.1 of the Secondment Policy, must consult with their manager before applying.

About You
The newly created role of EHCP Social Care Officer will play a vital part in supporting children and young people with Special Educational Needs and Disabilities (SEND), and their families, right at our front door and across Children’s Social Care teams. We look for individuals who are passionate about making a difference for children and young people with SEND and who embody our values and behaviours.
About The Team
You will be working within our Multi-Agency Safeguarding Hub (MASH) and collaborating across all Children’s Social Care teams. This role offers excellent opportunities for professional development and career progression.
Key Responsibilities
* Lead on social care assessments as part of the Education, Health and Care (EHC) Plan process.
* Work collaboratively with children, young people, families, schools, health professionals, and other partners.
* Ensure timely progression of EHC social care assessments and plans.
* Deliver effective interventions that support children within their families and improve outcomes.
