Community Respiratory Practitioner/Nurse

Job overview and responsibilities for a band 6 nurse/therapist to join the community respiratory team at Calderdale & Huddersfield NHS Trust and support the virtual ward agenda across both sites. The service provides specialist interventions for asthma, COPD, ILD, and Bronchiectasis, and works with Pulmonary rehabilitation to ensure all elements of treatment are delivered. The team also delivers the home oxygen service throughout Calderdale, both in a clinical environment and in patients’ homes. The service continually seeks new ways of working to support Calderdale’s population and collaborates with system partners to improve respiratory services.

The successful applicant will have or be willing to work towards a diploma/degree level qualification in respiratory disease and will be able to deliver high quality care with independent decision-making within the multidisciplinary team. The community respiratory service is committed to continuing professional development to support patient outcomes and high patient satisfaction.


Main duties of the job

* Act as a specialist resource for adult respiratory patients to assess physical and psychological needs and plan holistic care.
* Autonomously manage own clinical caseload and provide in-reach to Calderdale Royal Hospital to support virtual wards, admission avoidance, and Covid-19 early supported discharge.
* Use specialist knowledge to diagnose, assess and deliver evidence-based care through education and self-management advice.
* Collaborate with palliative care teams to develop advanced care planning for patients at the end of life.
* Monitor respiratory indicators, anticipate decline, and proactively manage to maximise wellbeing and independence.
* Contribute to the development of protocols, guidelines and policies with the team.
* Act as a focal point for support and advice, guiding patients and carers through the pathway.
* Provide telephone support, assessment and management advice in line with national and local guidelines.
* Participate in nurse-led clinics.
* Assess and implement patients’ need for long-term oxygen therapy and liaise with oxygen suppliers when prescribing and ordering oxygen.

Detailed Job Description And Main Responsibilities

* Support the delivery of education to the wider community nursing teams and primary care colleagues.
* Maintain effective links with the community and specialist respiratory services, including GPs, practice nurses, respiratory physicians, pharmacists, community matrons and respiratory clinics.
* Commitment to safeguarding and promoting welfare of children, young people and vulnerable adults.
* Support the vision, purpose and objectives of the community respiratory services.
* Support the Nurse Manager to ensure quality, performance, productivity, financial and contracting requirements are met.
* Contribute to the development of the community respiratory service through collaboration with peers and partners across community and specialist units.
* Be accountable for the management of patients in care, ensuring high standard, evidence-based clinical care from start to end of life.
* Support integrated patient pathways including care in clinics and domiciliary settings.
* Ensure governance compliance and up-to-date electronic patient records in line with trust policy.


Person specification

Qualifications / Training

* Essential: First level registration with NMC or AHP
* Essential: Professional qualification in respiratory care
* Essential: Nurse prescribing qualification or willingness to work towards
* Essential: Accredited spirometry training or willingness to work towards

* Teaching qualification


Knowledge, Experience & Expertise

* Experience of partnership and multidisciplinary working
* Understanding of local and national policy development for respiratory services
* Experience of working in a community setting
* Good clinical reasoning with complex respiratory patients
* Accountability for own work and ability to define others’ responsibilities
* Ability to maintain legible and accurate records
* Awareness of relevant national guidance and initiatives; recent respiratory/acute/community experience
* Understanding of clinical governance
