Main duties of the job General Clinical Responsibilities • To undertake psychiatric assessments and reviews of patients referred to the respective services, under indirect senior supervision. • To provide appropriate clinical cover in cases of emergency or urgent clinical need. • To support a seamless service and continuity of care, especially with the acute mental health services. • To support a patient-centred service, encouraging social inclusion and respecting diversity. • To work with other clinical staff in achieving high standards of assessment and care for all patients presenting to the service. • To adopt the Care Programme Approach and support integrated care pathways. • To carry out HoNOS scoring and PbR clustering in conjunction with the multi-disciplinary teams. • To observe the Trusts agreed policies and procedures. • To engage in regular consultant supervision of clinical practice as well as professional and educational development as appropriate. About us We have also been working with other local NHS Trusts across our region to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust - Hampshire and Isle of Wight Healthcare NHS Foundation Trust - to be formed by April 2024. People employed in our community, mental health and learning disability services by the Isle of Wight NHS Trust on 31 March 2024, will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) on 1 April 2024. Job responsibilities Key responsibilities: Assessing any patients admitted since Friday who have not yet seen anyone moresenior than a F2/SHO doctor; and making a management plan as appropriate. Assessing any patients suitable for discharge over the weekend as per establisheddischarge plans or the 72 hour assessment policy Providing advice and support to junior doctor on call Attending for S12 assessments (if suitably qualified) Attending any medical/psychiatric emergencies if requested to do so Middle Grade Doctor on call general roles and responsibilities They are fit for work and available, via the telephone numbers provided to the MH&LDMGoC administrator, throughout the duration of their on call shift. The MGoC must be onthe Isle of Wight throughout the duration of their on call shift. They advise the MH&LD MGoC administrator of any unavailability (booked leave, outsidecommitments, ill health) at their earliest convenience. Should they become unwell OOH,they must also contact the Consultant on call to advise them of this. That they have access to all the necessary Information Systems (I.S.) used by the Trustfor clinical work e.g. PARIS, e-Carelogic and JAC. If this is not the case, to take this upwith I.S. team, management and senior colleagues as soon as this is known, to remedythe situation. To collect the allocated on-call Bleep, Alarm and keys from the junior doctors/middlegrade office at the start of each shift and return them at the end of each shift. They make contact with the SHO on duty at the start of each on call period, and confirmhow they can contact each other. Review any outstanding tasks from the junior doctor handover (including looking at thejunior doctor handover log book in the junior doctor office) and any known work to bedone, with the junior doctor to prioritise these, according to clinical need and urgency.Also to identify tasks that should be performed by the MGoC rather than the junior doctor.Page | 22 Make contact with the Consultant on call at the start of each on call period, and confirmhow they can contact each other. Review any patients recently admitted, that have not yet been seen by a senior doctor(Middle Grade of above). This will require liaison with ward and junior doctor staff. To review patients as request by ward nursing staff and junior doctors, that require seniorreview. To review patients who are potentially suitable for discharge during the on call period, inkeeping with an established discharge plan made by the patients usual medical team. To act as Second Doctor for Mental Health Act Assessments (MHAA), if appropriatelytrained, and if this does not constitute a conflict of interests, as per the code of conductfor MHAA. If a MHAA request is made and the MGoC is not able to perform this duty, toliaise with the consultant on call / duty Approved Mental Health Practitioner (AMHP) sothat another doctor can be found promptly. The MGoC must be contacted in the event of any Serious or Major Incidents, includingbut not limited to medical emergencies, assaults on staff/patients, unexpected patientdeaths, and terrorism incidents. To relay this information to the Consultant on call andseek advice/support as required. To provide support and advice to junior medical staffand nursing staff as required. In the event that the junior doctor on call is taken ill or unable to complete their shift, ordoes not attend for their on call shift the MGoC will liaise with the Consultant on call todetermine to what extent it may be necessary for the MGoC to act down in accordancewith Trust Policy to meet clinical needs during the on call period. This should not be aregular occurrence, and the MGoC should not be required to routinely do this when thereare difficulties filling gaps in the junior doctor rota. Comply with reasonable requests by the Trust to complete Diary Card or similar suchexercises to properly assess the workload and nature of requests for MGoC involvementduring on call periods. Qualifications Essential • Has achieved MBBS or equivalent medical qualification • Foundation competencies or equivalent • Eligible for section 12 approval (requires 4 years experience in Psychiatry - can include F2 or equivalent experience) • 5 years full-time postgraduate training (or its equivalent gained on a part-time or flexible basis)- 3 years of which as a CT1-3 in Psychiatry or equivalent experience/ competencies.
