Essential
RGN or equivalent AHPclinical qualification
Degree or masters or willingness to work towards
Evidence of relevant and on-going professional development
Heart failure diploma or equivalent M level module
Experience of long term condition case management
Experience of working in collaboration with other professionals across organisational boundaries.
Leadership experience
Basic IT skills; experience of electronic patient records
Ability to demonstrate effective verbal and written communication skills.
Ability to co-ordinate and direct programmes of care working collaboratively and autonomously.
Mentorship skills
Desirable
Independent/ extended or supplementary prescribing
Extensive experience of working with older people or those with long-term conditions, Eg Respiratory Diabetes .
Experience of developing and implementing creative/ complex care packages
Recent experience of working in the primary care setting
Experience of managing change and service improvement.
Experience of managing psychological impact of heart failure
