Overview

West London NHS Trust is seeking an experienced and motivated pharmacist to take on the role of Lead Integrated Care Pharmacist within the Hammersmith & Fulham Community Independence Service (CIS). This is a key leadership position within a multidisciplinary team delivering integrated, person-centred care to patients in their own homes. The post holder will provide strategic and clinical leadership for the pharmacy component of the service, contributing to medicines optimisation and patient safety across the borough.

This role is for a clinically skilled pharmacist with an interest in integrated care, medicines management, and collaborative working across health and social care boundaries. The successful applicant will demonstrate excellent verbal and written communication skills and be a confident leader with extensive clinical knowledge in medicines management and medicines safety.


Main responsibilities

* To provide a high-quality patient-focused clinical pharmacy service in an Integrated Care setting at West London NHS Trust.
* To lead on the delivery of a clinical pharmacy service across the Community Independence Service.
* To carry out domiciliary assessments and medication reviews in identified high-risk patients with complex medicines needs.
* To work closely and collaboratively with the ECP pharmacy leadership team and service.
* To ensure the successful implementation and roll out of Trust procedures, quality improvement projects and national legislation.

Qualifications and experience

Essential

* Master7s Degree in Pharmacy or equivalent
* Clinical pharmacy certificate (part 1 of clinical diploma) or demonstrable significant equivalent experience
* Clinical diploma (part 2 of clinical diploma) or near completion, or demonstrable significant equivalent experience
* Registered member of the General Pharmaceutical Council (GPhC)

Desirable

* Non-Medical Prescriber
* Management qualification or significant relevant experience

Experience

* With relevant and significant pharmacy experience relevant to the post
* Significant experience in providing clinical pharmacy to medical wards in acute settings (hospitals)
* Significant experience in providing medication reviews in older people and medicines optimisation, including structured medication reviews
* Delivering direct pharmaceutical care to patients with long-term conditions
* Experience of medicines management contributions and MDT-related patient care

Desirable experience

* Experience of project management and managing change
* Experience working across wider health and social care integrated settings/ICBs or partnerships and in multidisciplinary teams
* Led an audit or research/project or quality improvement initiative
* Management skills and experience (recruitment, appraisal, training, leadership, managing sickness absence)
* Experience in an integrated care environment

Knowledge

* Knowledge of national clinical guidance (e.g. NICE, NHS England, MHRA)
* Knowledge to develop and implement policies and procedures
* Desirable: knowledge of the integrated care pharmacy service

Skills and other requirements

* Leadership skills and ability to lead a team or service
* Self-motivation, reflective practice and continuous improvement
* Organisational and prioritisation abilities
* Excellent interpersonal and influencing skills
* Good time management, problem solving, and IT proficiency (including Microsoft Office, Systm1, EPMA)
* Ability to convey information to patients who may be distressed or have barriers to understanding
* Desirable: valid UK driving license
