Job Overview
* The core role of the Advanced Nurse Practitioner will be to autonomously examine, diagnose and treat patients.
* The post holder will work collaboratively with and support members of the multidisciplinary team in the delivery, development and management of complex health and social care issues as required across Gwent Rapid Frailty Response services. In particular they will be responsible for ensuring the timely provision of a high standard of nursing assessment, advice, planning, implementation, training support and evaluation of care taking into account the social context in which a person lives. They will therefore be engaged in the planning and delivery of collaborative health and social care across community and hospital settings as appropriate.
* To promote evidence-based practice and act as an accountable role model.
* Ensure patient pathway is effective and timely.
* The advanced practitioner will be responsible for the provision of effective, efficient and appropriate high quality nursing care.
* Act at all times in accordance with the NMC Professional Code of Conduct.
* Advanced Nurse Practitioners are highly experienced and educated members of the care team who are able to diagnose and treat healthcare needs or refer to an appropriate practitioner if required’ NMC definition (2005)
Main duties of the job
* The post holder practices advanced nursing within the Framework for Advanced Practice in Wales 2010. Core service delivery is covered by four pillars of practice, Clinical Practice, Management and Leadership, Education and Research.
* The post holder is expected to monitor standards of nursing care, patient experience and patient safety and take appropriate action to maintain and improve them
* Ensuring that all delegated activities are in line with the NLIAH (2010) All Wales Guidelines for Delegation and/or HEIW Frameworks.
* The post holder is required to be registered with the Nursing and Midwifery Council.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
