Overview
SWUFT Out of Hospital Division has an exciting opportunity for a full-time Professional Lead (6 month fixed term or secondment) to work within the South Warwickshire Community Response Teams. The teams are based at Whitnash Lodge, RLSRH, Leamington Spa and Hathaway, Stratford upon Avon Hospital. The role involves overseeing the Warwick and Stratford community response teams (CRT), facilitating discharge of patients from acute hospitals with a South Warwickshire GP, assessing patients in their own homes, preventing admission and providing palliative care support.
The post focuses on assessment, intermediate interventions and reviews that contribute to patient rehabilitation and assessment of care needs. The team educates and encourages patients in activities of daily living, administration of medication, and therapeutic goals.
The role offers opportunities to develop knowledge, competence and managerial skills in a dynamic health and social care environment, collaborating with colleagues across the locality, allied health professionals, place-based teams, urgent community response leads, and specialist services, as well as liaising with ICS partners within the acute Trusts, local authority, clinical commissioners and 3rd party care providers.

Main duties of the job
The main purpose is to work with Integrated Teams within a defined locality, developing services and multi-disciplinary teams through clinical leadership and operational management within a clinical governance framework, in line with the overall strategy for South Warwickshire NHS Foundation Trust. The role will involve deputising for the Locality Manager to ensure continuity in the operational management of teams.

Job description
Key Responsibilities
* Oversee clinical services focusing on people with complex needs, ensuring personalised care planning and quality outcomes.
* Directly manage and develop multi-disciplinary team working within localities through leadership, teaching and assessment.
* Assist in redesigning care services, engaging stakeholders, incorporating Productive Community Working principles.
* Ensure efficient use of resources within the span of control to provide high standards of care.
* Develop and maintain joint working initiatives with other team members and voluntary/statutory agencies to enhance services.
* Ensure performance indicators and activity targets are achieved.

Clinical Role
* Maintain professional knowledge and clinical practice per NMC Guidelines (450 hours registered practice in last 3 years and 35 hours CPD in last 3 years).
* Practice can be met through administrative, supervisory, teaching, research, managerial roles or direct patient care.
* Oversee complex cases, assist the team when staff are short, manage discharges and scheduling, and advise on self-management opportunities.
* Initiate and participate in clinical audit; support research programmes and evidence-based practice.
* Facilitate development of evidence-based practice and manage mentorship/preceptorship within the MDT.
* Ensure access to appropriate specialist advice and clinical supervision for Child/Vulnerable Adults Protection; facilitate timely caseload reviews per policies.
* Empower patients to participate in their own care.
Management Role
* Ensure efficiency in resource use and monitor local budgets; authorised signatory and adherence to Standing Financial Instructions.
* Manage Health, Safety and Security and risks; ensure policy compliance and incident management.
* Support workforce planning and staff development, recruitment and retention; manage sickness/absence and respond to complaints/incidents.
* Hold staff to account for conduct and performance; deputise for the Locality Manager in their absence.
* Conduct annual Performance Development Reviews and oversee induction, mandatory training and governance training.
* Review Scheduling Tool and develop local induction processes.

Qualifications
* Registered in Health or Social Care Professional Qualification
* First-level Degree or equivalent
* Recent CPD
* EDCL 1 or equivalent experience
* Postgraduate knowledge in a specialist area (e.g., paediatrics, health visiting, public health)
* Management qualification or equivalent
Experience
* Substantial experience at team leader or equivalent in health or social care
* Significant community experience at band 6 level; 1 year at band 7
