Job Overview
JOB PURPOSE
The post holder will be responsible for working in Health and Justice (H&J) Services, depending on skill and service need. Health and Justice is not a treatment service but a service for identification, assessment, and short-term support.
There will be Health and Justice Practitioners within Custody Suites, Courts, and Prisons (RECONNECT) to identify and assess vulnerable individuals. They use assessments to make appropriate referrals for treatment and support, and ensure criminal justice practitioners are notified of specific health requirements and vulnerabilities, which can influence decisions about charging and sentencing.
The post holder will provide short-term support to vulnerable individuals upon leaving Custody or Court, working closely with them to prioritize needs and plan actions. This may include supporting referrals to specific services and assisting individuals to attend initial appointments.
The role involves ongoing assessment and access to a wide, sometimes complex, range of care services across an ageless Service.
The post holder will carry out assigned tasks as part of the Health and Justice Service.
Main duties of the job
* Working with service users and their carers to promote self-determination, choice, and autonomy.
* Establishing therapeutic relationships with service users and relatives/caregivers.
* Participating in setting quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
* Assessing individual care needs, referring to appropriate agencies, involving individuals in planning, and providing short-term interventions to encourage engagement with services.
* Using agreed outcome measures to review the effectiveness of services.
* Participating in service development to ensure clinical governance is implemented at team level.
* Ensuring the aims of the service are met and identifying factors that inhibit achievement.
* Remaining up-to-date and compliant with mandatory training requirements.
* Supporting service in various community settings across the Black Country, with the base at Crescent House, Bilston.

Person specification
Qualifications & Training
Essential: Competent in literacy and numeracy, basic knowledge of physical/mental health and rehabilitation, computer literacy.
Desirable: NVQ Level 2 Health & Social Care or equivalent.
Experience
Essential: Experience working with individuals with complex health and social care needs, knowledge of the Criminal Justice System, experience with care or rehabilitation plans.
Skills, Knowledge & Abilities
Essential: Awareness of vulnerabilities, rapport-building skills, professionalism, flexibility, and a driver’s license with business insurance.
