Responsibilities

* Organising and participating in the delivery of high standards of evidence-based nursing care.
* Carrying out planned direct patient care to a high standard without direct supervision, in accordance with established nursing procedures and policies.
* Contributing towards health promotion for all patients by providing education, support and advice as required, ensuring referral to appropriate professionals or agencies as needed.
* In the absence of senior staff, delegating and supervising appropriate work to other qualified staff, healthcare support workers and students.
* Co-ordinate discharge planning in partnership with the multidisciplinary team where relevant.
* Report significant changes in a patient\'s condition or situation to the appropriate members of the multidisciplinary team.
* Take responsibility as a team leader for the delivery of care in a defined area.
* Ensure safe ordering, administration and custody of patients\' medication according to Trust policies and procedures.
* Be responsible for emergency situations and be familiar with emergency procedures including fire and resuscitation; supervise junior staff accordingly when needed.
* Contribute to health promotion for all patients by providing education, support and advice and ensure appropriate referrals.
* Be responsible for admitting, assessing and preparing patients undergoing treatment or investigations where relevant.
* Escort patients to other wards or departments as required.
* Act to ensure that patients are treated with courtesy, their individual needs are respected and confidentiality is maintained at all times.
* liaise effectively with other disciplines or departments as appropriate.
* Be able to act in emergency situations in line with Trust policy and assist with the supervision of junior staff during these events.
* Ensure readiness for use of special equipment and availability of nursing supplies in conjunction with senior ward nurses.
* Report and document any broken or damaged equipment according to Trust policy.
* Monitor stock levels and promote efficient use of supplies.
* Assist with ward administration and sickness reporting to maintain skill mix.
* Coordinate with the multidisciplinary team to support discharge planning where relevant.
* Be aware of manual handling regulations and ensure patient handling needs are met by junior staff and students.


Working Conditions / Effort

* Staff may be required to work in any Trust site depending on service demands.
* Workloads may be unpredictable; weekend and night working may be required unless otherwise arranged.
* Walking or standing for the majority of shifts; manual handling of loads and patients using appropriate aids.
* Potential contact with blood and bodily fluids; handling distressed or challenging patients and relatives as needed.
* Ability to concentrate and manage multiple tasks and delegation during shifts; respond quickly to emergencies.
