We have an exciting new opportunity to recruit a permanent Specialist Mental Health Practitioner to work in the Shore Medical Primary Care Network, which comprises GP Practices based in east Dorset. The successful candidate will be an experienced Mental Health Practitioner with experience of working in a Primary Care and/or Community Mental Health Team setting.

Applications from candidates that require current Healthcare worker sponsorship to work in the UK are unfortunately ineligible to apply for this position. This new role will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care.
The intention is to focus services around local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve. This practitioner role will provide targeted care according to the needs of the population by supporting the PCN clinical strategy. The practitioner will facilitate onward access to other secondary care-based services, third sector services and link in with existing support services via the Primary Care Network.
Responsibilities
* Provide targeted mental health care in a primary care setting as part of an integrated model with secondary mental health services.
* Collaborate between Primary Care and Secondary Mental Health Services to support local communities and GP practices.
* Facilitate onward access to secondary care services, third sector services, and existing support services via the Primary Care Network.
* Support the PCN clinical strategy to meet population needs.
Requirements and Benefits
* Post is full-time 37.5 hours, with consideration given to job share or part-time working.
* Enhanced DBS check required; the Trust will cover the cost. Applicants who subscribe to the DBS update service and present a valid DBS certificate may not require a new check.
* Experience in Primary Care and/or Community Mental Health Team settings is preferred.
* Employment in this post requires eligibility to work in the UK; sponsorship restrictions apply.
