Community Reablement Support Worker We are seeking a compassionate and proactive support professional to help people regain independence and confidence after returning home from hospital.Position: Community Reablement Support WorkerLocation: Salisbury, WiltshireSalary: £29,979 to £31,458 per annum inclusive of 10% unsocial hours enhancementHours: Full time, 37 hours per weekContract: PermanentAbout the RoleAn opportunity has arisen to join a community-based support team helping people safely transition from hospital back into their own homes. Working across the local community, you will provide practical and person-centred support to individuals recovering from illness, injury, or changes in circumstances, enabling them to regain confidence and independence.This is a varied and rewarding role where no two days are the same. You will work closely with colleagues and healthcare professionals to assess needs, provide support, and develop personalised care plans that promote independence and wellbeing.The role requires flexibility to work early mornings, evenings, weekends, and bank holidays as part of a rota.Key responsibilities include:Supporting individuals in their own homes following hospital dischargeCarrying out initial assessments and contributing to support planningProviding personal care and assistance with daily living activitiesSupporting individuals with meal preparation and maintaining independenceWorking collaboratively with healthcare professionals and wider support teamsMaintaining accurate records and following safeguarding proceduresDelivering compassionate, respectful, and person-centred support at all timesAbout YouYou will be caring, reliable, and passionate about supporting people to live independently. You will enjoy working both independently and as part of a team and be able to respond calmly to changing situations.You will ideally have:A Care Certificate, Level 2 Health and Social Care qualification, or equivalent experienceExperience supporting older people and/or people with learning disabilitiesAn understanding of safeguarding and community care principlesGood communication and interpersonal skillsStrong organisational skills and the ability to prioritise workloadA compassionate and empathetic approachA flexible and adaptable attitudeAbout the OrganisationThis organisation is committed to supporting local communities through high-quality care and reablement services.
