Dutiesand responsibilities:
Managementof patient Health and Illness
To deliver a high standard of patient care asan Advanced Care Practitioner in general practice, using advanced autonomousclinical skills, and a broad and in-depth theoretical knowledge base.
Provide a first point of contact within thePractice for patients presenting withundifferentiated, undiagnosed problems, makinguse of skills in history taking, physicalexamination, problem-solving and clinicaldecision-making, to establish a diagnosis andmanagement plan.
Instigate necessary invasive and non-invasivediagnostic tests or investigations and interpret findings/reports.
Where the post-holder is an independentprescriber: to prescribe safe, effective andappropriate medication as defined by currentlegislative framework.
To prioritise health problems and interveneappropriately, including initiation of effective emergency care.
Promote health and well-being through the useof health promotion, health education,screening and therapeutic communicationskills.
To clearly communicate the health status ofthe patient using appropriate terminology,technology and format.
Provide safe, evidence-based, cost effective,individualised patient care.
To work within the Practice guidelines,policies and protocols.
To maintain accurate and contemporaneousrecords, utilising computer systems whereappropriate and consider the CaldicottPrinciples and GDPR Regulations in relation to all data handling.
