Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help homebound patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. We serve patients from rehabilitation after illness or injury to chronic disease management, working to optimize independence and quality of life. Therapists function both independently and as part of an interdisciplinary team, delivering care that is deeply personalized to each patient’s home and community environment.
Responsibilities
* Provide physical therapy services to patients in their place of residence with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes (patients may range in age from birth to over 100).
* Develop a plan of care that is patient- and family-focused, and institute standardized interventions for goal-oriented outcomes.
* Perform daily documentation that is accurate and timely.
* Apply standard treatment procedures and provide teaching to patients and caregivers.
* Collaborate with the patient, family, physician, and other allied health professionals to identify needs and coordinate services to meet those needs.
* Travel within the Mass General Brigham Healthcare at Home geographic area as required.
Qualifications
* One year of experience in acute care, rehab, or homecare as a Physical Therapist.
* Registered with the Massachusetts Board of Registration.
* Excellent verbal, written, and electronic communication skills.
* Computer proficiency and ability to learn new software.
* Ability to prioritize work and adapt to changing situations.
* Travel required within Mass General Brigham Healthcare at Home’s geographic area.
