Job overview
Derbyshire Health Care Foundation Trust has a bespoke Older Adult service and a robust pathway which includes inpatients, Crisis, psychiatric liaison and Community Mental Health Team.
We are advertising a 1 year fixed term contract into the Cubley Court dementia wards. You will be working along side an experienced and skilled team and an established consultant into the 36 beds across the two wards.
Main duties of the job
It is a great time to join Derbyshire Healthcare. As the Consultant Psychiatrist (Older Adults), you will work along side the existing staff on the ward as well as the established and highly skilled consultant.
You will be responsible for a cohort of older adult patients with a clear dementia diagnosis and their families and friends to reach the best outcomes for the next part of their journey. This is an opportunity to control in-patient admissions and support flow and discharge working to reduce length of stay.

While primarily responsible for delivering a quality clinical service, as Consultant Psychiatrist you will also be expected to be actively involved in the strategic development of the team and broader services, and to work with the team manager and service managers in helping to steer the development of the service in line with the strategic direction of the organisation.
You will provide holistic care with an emphasis on medical assessment and diagnosis, pharmacological treatment and medicines management, support and consultation in the management of complex patients in collaboration with the Inpatient and Crisis Teams as part of the multi-disciplinary care of patients. You will be focused on the most complex cases and will also be available to provide consultation and advice to other team members.
The older adult service line is well established within the Trust and there is a clear pathway for patients. The In-Reach Home Treatment Team adheres to national benchmarks for fidelity of care in Crisis services. This includes diagnosing well and supporting people to maintain well in their community.
The In-Reach Home Treatment Team acts as the gate-keepers for Functional mental health beds and most admissions are facilitated through this team. Bed occupancy and length of stay have all been positively affected by the development of this team. The team also works proactively to support early discharge.
