LOCATION: Home Working/Head Office – Travel expected around the regions of Northamptonshire and Nottingham & Nottinghamshire. Ad hoc additional travel may be required.

HOURS: 37.5 Hours per week, Fixed-term contract 12 Months

LWTC aims to deliver high-quality, efficient, cost-effective services, targeted at specified requirements but sympathetic to the changing needs of its clients, commissioners, and the local community.

Job Summary:

The Prevention Facilitator will be instrumental in supporting the effective delivery and optimisation of the NHS Healthier You: Diabetes Prevention Programme (DPP) and the LWTC Weight Management Programme. This role involves engaging with primary care stakeholders, analysing patient data to drive informed decisions, and implementing strategies to enhance Primary Care and Public patient engagement, and referral quality to the LWTC programs. The Engagement Officer and Prevention Facilitator role is focused on the regions of Northamptonshire and Nottingham & Nottinghamshire however will support the engagement activities in other Midlands Contracts and may be requested to support nationally on an ad hoc basis.

Key Responsibilities:

Stakeholder Engagement and Awareness Building:

·Attend primary care meetings and forums, such as GP forums, Practice Nurse and Practice Manager meetings, to raise awareness of the NHS DPP and LWTC Weight Management Programme.

·Deliver presentations and informational sessions to primary care providers, explaining the programmes objectives, eligibility criteria, and referral process.

·Foster collaborative relationships with General Practice and Primary Care Networks to ensure consistent and appropriate patient referrals to the NHS DPP and LWTC Weight Management Programme.

Content Development, Promotion and Insight Gathering:

· Develop and distribute promotional newsletters and data-based information packs to provide update stakeholders on programme developments and successes.

· Attend local health and wellbeing events to promote and raise awareness of the NHS DPP and LWTC Weight Management Programme.

·Support LWTC National Team

·Collect and collate case studies to support programme evaluation, promotion, and continuous improvement.

Referral Quality Management:

· Identify barriers and challenges associated with the quality of referrals from General Practice to the NHS DPP.

· Offer guidance and support to primary care referrers on optimizing referral pathways and increasing referral rates.

· Assist practices in utilising NHS DPP electronic templates and integrating patient outcome recording into clinical systems (e.g., EMIS/VISION Web) to streamline referral processes.

· Promote, coordinate and oversee localised referral generation projects

Data Analysis and Insights Gathering:

· Gather and analyse data related to patient engagement and non-engagement with the programme.

·Contact patients who accept the programme but do not convert to Initial Assessment (IA) to understand underlying barriers.

·Reach out to patients who drop off post-MS1 to identify retention issues and factors contributing to disengagement.

Communication and Collaboration:

· Communicate identified patient issues and data findings to the LWTC Management Team and relevant stakeholders.

·Generate actionable recommendations for LWTC, Primary Care, and the Steering Group based on insights obtained.

·Collaborate with LWTC Compliance and Liaison Officers to support smooth running of the programmes in each region.

Main Areas of Influence:

· General Practice

· Senior commissioners within ICBs, Local Authorities

· Primary Care Networks

· Other health providers and partners

· Other stakeholders and voluntary sector providers

· Local patient participation groups

Qualifications / Education

* A bachelor's degree in a relevant field (e.g., Public Health, Healthcare Management) is essential.
* At least three A-Levels or equivalent is essential.
* A Health and Social Care Level 5 diploma or equivalent/relevant experience is desirable.

Experience

* Experience in engaging and supporting General Practice in referral generation is essential.
* The ability to analyse and interpret statistical performance information and take actionable measures based on insights is essential.
* Previous experience working within primary care or public health to deliver health and wellbeing services is essential.
* Experience collecting insights on barriers/issues among patients and healthcare professionals and reporting findings is desirable.
* Familiarity with General Practice clinical systems including EMIS, Vision, and System One is desirable.

Knowledge

* Knowledge of local and national NHS issues, or the ability to quickly learn about detailed and complex issues, is essential.
* Familiarity with the NHS Data Protection and its objectives is desirable.
* Understanding of primary care operations and the roles essential for referring patients to services is desirable.
* Knowledge of referral pathways and challenges related to referrals is desirable.

Key Skills / Abilities

* Strong communication and presentation skills, with the ability to engage and influence diverse stakeholders, are essential.
* The ability to liaise with General Practice staff in a sensitive and tactful manner, considering the pressures faced by primary care, is desirable.
* Clear and concise writing style for effective communication is essential.
* Excellent prioritisation skills and the ability to meet deadlines under pressure are essential.
* The capacity to work independently as well as collaboratively within a team is essential.
* A commitment to maintaining confidentiality is essential.
* Flexibility to work outside normal hours and the willingness to travel across the region as required is essential.
* Possession of a car and a full clean driving license is essential.

Proficiency in computer applications including Microsoft Word, Excel, Internet, and email, with good keyboard skills, is essential.

Pay: Up to £26,000.00 per year
