Overview
The team will consist of a variety of highly skilled professionals including nursing, support staff, medical, psychology and occupational health colleagues. You will work closely with all of them to ensure the care we provide is not only outstanding but that the team is well developed and supported.
Responsibilities
* Contribute to a reduction in physical health illness in those service users with complex mental health needs.
* Increase physical health screening in those service users with complex health needs.
* Ensure that the physical health needs of the service users are met through sharing information with the service users\' primary care team.
* Achieve a high level of patient and professional satisfaction.
* Promote self-management, self-care and recovery.
* Maintain the independence of service users.
