An exciting opportunity has arisen for an experienced and motivated Nurse Practitioner to join our friendly and supportive team. We are looking for a dedicated professional who can work autonomously and collaboratively to deliver high-quality, person-centred care.
You will play a key role in managing long-term conditions including:
1. Diabetes
2. Hypertension
3. Heart Failure
4. COPD
5. Asthma
Key Responsibilities
Assess, diagnose, and formulate treatment plans for acute and chronic illnesses
6. Conduct telephone and in-person reviews for patients with chronic conditions
7. Manage prescriptions, results, and referrals
8. Review Medlink questionnaires and interpret test results
9. Educate patients on preventative healthcare and lifestyle changes
10. Perform core nursing duties including injections and cervical screening
11. Collaborate with the wider clinical and administrative team
12. Maintain accurate and up-to-date medical records
Essential Requirements
NMC Registration
Previous General Practice experience
Experience in Minor Illness Treatment and Chronic Disease Management
Registered Non-Medical Prescriber
Evidence of continuous professional development
13. Computer literate
14. Excellent communication and interpersonal skills
15. Ability to work under pressure and multitask in a busy, changing environment
Desirable Skills & Qualifications
Experience using Medlink
Independent Nurse Prescriber qualification (preferred but not essential)
Strong organisational skills
Ability to work independently and as part of a multidisciplinary team
