Overview
Join to apply for the Triage Lead-Senior Nurse Practitioner role at Sussex Partnership NHS Foundation Trust in Hastings and Rother NMHT. The SNP will be part of the clinical leadership team within the Assessment and Treatment Service (ATS) and take a lead with the Triage Team, ensuring the safe delivery and management of referrals into ATS.
The post is front-end to our delivery service, requiring strong communication, collaboration, and relationship-building with service users, families and carers, private providers and local mental health care teams. You will be joining a committed and dynamic team and will be fully supported to develop innovative ways of working to deliver safe, responsive mental health services.
Responsibilities
* Triaging and managing referrals into the ATS within agreed time scales and aligned to clinical need.
* Provide strong clinical leadership and expertise to the multidisciplinary team and NMHT, helping maintain high standards.
* Work closely with the medical team to ensure effective patient flow and discharge management.
* Lead by example, modelling Trust values and behaviours, and fostering collaboration across partners and services.
* Engage with stakeholders including service users, families and carers, private providers and local mental health care teams to support comprehensive care.
* Contribute to the development of new ways of working to enhance safety and responsiveness of services.
* Demonstrate substantial clinical experience in a community mental health setting and use initiative, creativity, and flexibility in problem-solving.
Qualifications and Skills
* Substantial clinical experience within a community mental health setting.
* Demonstrated leadership acumen and ability to work collaboratively across partners.
* Strong communication and interpersonal skills; ability to build trusting relationships with families, carers and colleagues.
* Ability to work flexibly in multidisciplinary and multiprofessional teams and across a range of settings.
* Knowledge of mental health services and third-sector organisations.
Hours and Location
Currently 9am - 5pm, Monday to Friday. You will be expected to work across the Hastings and Rother NMHTs.
