Are you an experienced nurse focused on delivering exceptional care to children, young people and families? Join EACH's specialist Symptom Management Service and make a meaningful difference every day.

You'll provide advanced, holistic palliative and symptom management care to babies, children and young people with complex, life limiting conditions across Cambridgeshire, Norfolk, Suffolk and parts of Essex. Working both independently and as part of a multi-professional team, you'll offer expert clinical assessment, guidance and support in a range of settings, including home, hospital, hospice and virtual consultations.


Main duties of the job

Provide advanced level symptom management, care and support to children with life-threateningillnesses and complex health care needs living in Cambridgeshire, Norfolk, Suffolk and NorthEast and West Essex as part of the EACH Symptom Management Nursing Service.

Participate in an on call rota, triaging telephone calls and undertaking face to face visits to deliverthe symptom management service across the whole EACH catchment area as required.

24/7 service provides support for designated children and their families at home and clinicalleadership advice and support to children receiving core hospice services across EACH.

The team, consisting of Clinical Nurse Specialists and a Matron acts as a point of contact forsymptom control and management issues for professionals and teams within the regionalmanaged clinical network (MCN); and with the increasing capacity within the tertiary basedSpecialist Palliative Care Team (RAaFT) based at Cambridge University Hospital.

Job responsibilities

* As an autonomous practitioner provide holistic and evidence based advanced level nursing careto children and young people with complex palliative care, pain and symptom control needs asan integral part of the EACH multi-professional core service teams, MCN and RegionalSpecialist Childrens Palliative Care Team. (RAaFT)
* Undertake clinical assessment / consultation using face to face, telephone and virtual ITmethods as required.
* Manage a caseload within the EACH catchment area.
* Act as a point of contact for symptom control and management advice for children and youngpeople across the continuum of care in the full range of settings (home, hospital, hospice,school, residential placement).
* Develop and maintain specialist level palliative care knowledge and skills across a range ofprocedures and practices including medicines management.
* Participate in the 24/7 on call rota, triaging telephone calls and undertaking face to face visitsto deliver the symptom management service across the EACH catchment area as required.Additional hours worked when on call are over contracted hours and will be paid as overtime.
* Work collaboratively with the on call medical practitioners in the MCN / Specialist Team whoprovide specialist level telephone advice to the service outside office hours.
* Collaborate with all members of the EACH multi-professional team, MCN / Specialist Team tocoordinate safe discharge for end of life care.
* Provide clinical expertise, advice, support and training opportunities to health, social care andeducation sector professionals throughout EACH and the Region to ensure a coordinated multidisciplinary approach to the provision of palliative care for children and young people.
* Develop, review and evaluate comprehensive symptom management plans, coordinating andsupporting their implementation and use within EACH core service teams and the Region.
* Provide clinical advice, expertise and support for care coordination teams in EACH andundertake a lead professional role when agreed with the multi-disciplinary team within theMCN and Specialist Team.
* Lead or support clinical audit and evaluation of ways of working and service efficacy, supportingclinical quality initiatives as agreed with the Matron and Head of Nursing
* Act as an independent prescriber within scope of professional practice (those with relevantqualification).
* Demonstrates advanced communication skills including the ability to have difficult or complexconversations with child, family and carers.
* Participate in clinical audit and evaluate ways of working and service efficacy, supportingclinical quality initiatives as agreed with the Matron and Head of Nursing


Qualifications

* Current UK NMC Registration NMC Registered Nurse (Child, Adult or Learning Disability with clinical experience) with minimum of 3 years experience of at least band 6
* Knowledge of current legislation relating to children & young people including best practice in safeguarding children
* IT literate (e.g. use of virtual platforms for remote clinical assessment / consultation, word processing and E-Mail)
* Detailed knowledge and understanding of children's palliative care and recent initiatives in children's health and social care
* Highly specialist standard of clinical and theoretical knowledge in childrens palliative care and symptom management
* Advanced level clinical assessment skills
* Independent and supplementary prescribing qualification
* Professional knowledge acquired through medical degree or diploma or other clinical professional training qualification
* Professional training in the field of paediatric palliative care.

* Highly developed written and advanced oral communication skills including the ability to have difficult conversations with child, family and carers
* Evidence of participation in research and practice development
* Ability to reflect on practice and positively receive constructive feedback to improve professionally
* Evidence of child/ young person/ family member teaching and information giving
* Evidence of delivering clinical teaching to professional and carers
* Evidence of facilitation/ negotiation skills
* Evidence of leading innovation and managing change/ service development
* Evidence of supporting others to develop knowledge and skills


Other requirements

* Current full, valid driving license and own transport
* Provide on call and on stand by clinical care i.e. able to respond out of hours as part of the clinical rota


Experience

* Significant post-registration experience in paediatric services, working at Band 6 or above
* Significant post registration experience with demonstrated experience of delivering care to children and families in a health care setting
* Experience of working with children and young people with complex and palliative care needs
* Experience of using advanced clinical assessment & judgement to interpret clinical data to inform decision making
* Evidence of working within a multidisciplinary team
* Evidence of providing mentorship to staff and students
* Experience of managing staff and resources on a day to day basis, including clinical supervision, mentoring and appraisal
* Experience of providing community care services
* Experience of using System One electronic care record system
