Haematology Macmillan Care support worker
The Haematology department is looking for an enthusiastic, forward thinking care support worker to join the team. The care support worker will work with the Haematology CNS team to provide care to Haematology patients. The care support worker will coordinate care for people with non-complex care needs, joining up services, coordinating information and communication between caregivers, treatment providers and working with patients living with and beyond cancer and their families to create a seamless experience of care.
Responsibilities
The Haematology Macmillan care Support Worker will be responsible and accountable for their practice and behaviour under the guidance and supervision of a registered practitioner and be a member of an existing team. Under the guidance and supervision of a registered practitioner:
* Triage incoming calls and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the team for assessment and review.
* Coordinate the care for a defined group of patients assessed by a registered practitioner as having level one care needs for supported self-management.

Person Specification
Qualifications
* NVQ4 or NVQ3 plus additional training to diploma level equivalent.
* GCSE English Language and Mathematics (Grades A-C) or equivalent
* Evidence of continued role development
* Willingness to undertake Macmillan learning and development courses
Experience
* Experience of working with multiprofessional team
* Evidence of good communications skills & use of data management
* Understanding of Macmillan Cancer Support and its role across the UK.
* Experience in coordinating a patient workload
