Community Staff Nurse - Lancaster

Lancaster District Nurses has an exciting and rare opportunity to recruit to a permanent 22.5 hour post per week.
Lancaster District Nurses are looking for an enthusiastic, hard working and highly motivated staff nurse, who is excited to work in an evolving district nursing service. The successful applicant will be committed to working closely with health and social care colleagues, to provide high quality care to patients with a wide range of medical conditions in the community setting.
This role includes working flexibly over 7 days including weekends and evening shifts.
Applications from student nurses due to qualify and newly qualified staff nurses are welcomed from those who wish to develop within the scope of Community Nursing.
Main duties of the job
The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programmes of care for patients and carers. The district nurses work a 24hr service and there is an expectation that you may be required to support this to ensure service delivery.
You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting.
You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard. Staff will have access to regular clinical supervision, training and Occupational Health services.
A full UK driving licence is required to carry out this role.
About us
We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI) and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
Qualifications
* Registered Nurse (on the relevant part of the register for the post)
* Mentoring and assessing qualification or working towards or equivalent
* Post registration clinical course appropriate to area or willingness to undertake further training relevant to the post - and to use and maintain these skills in the delivery of care
* ECDL or equivalent

* Evidence of Professional and personal development via a portfolio
* Experience of establishing effective communication
* Working within a multidisciplinary team
* Experience of implementing change
* Previous experience within designated areaExperience of mentoring and assessing members of the team
* Experience as shift team leader
