What you will do:
* Collaborate closely with older adults and those with frailty or complex needs to develop care plans that promote their independence and well-being.
* Conduct strengths-based assessments and craft effective care and support plans tailored to individual needs.
* Work alongside multi-disciplinary professionals including health specialists, hospital teams, and local community agencies to deliver seamless support.
* Facilitate safe hospital discharges and interventions that prevent unnecessary admissions, ensuring continuity of care.
* Lead on Mental Capacity Act assessments and safeguard investigations as required to protect individuals’ rights.
* Champion the values of independence, resilience, and high-quality outcomes for adults and their families.
What we’re looking for:
* A qualified Social Worker with SWE registration.
* Experience in adult social care, ideally with an understanding of frailty, hospital discharge processes, or intermediate care pathways.
* Proficiency in risk assessments, safeguarding practices, and crafting person‑centred care plans.
