Overview

Job summary: 37.5 hours per week

An exciting opportunity has arisen to be part of the Integrated Community Stroke Rehabilitation Team in Barnsley. The service provides care to patients following a stroke in both inpatient and community settings. It enables patients to access rehabilitation therapy upon discharge home, with the same intensity and expertise as in hospital. The Integrated team allows for stroke‑specific therapy to be continued as required for a maximum period of 6 months.

Successful candidates will work as part of a multi‑disciplinary team of nursing and allied health professionals in both community and inpatient settings, carrying out designated duties in a competent manner. It is essential that candidates have the ability to use their own initiative, have good communication skills and are able to manage their time effectively. The use of a vehicle to undertake any necessary travel in connection with the duties of the post is essential.

Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000, as required by the UK Visas and Immigration. For Band 3 roles, only full‑time positions at the top of the pay scale meet this threshold. Therefore, sponsorship may not be possible depending on salary increment placement and working hours.

Main duties of the job


Responsibilities

* As Rehabilitation Assistant you will: work as part of a Multi Professional Team in an inpatient and community setting providing the highest standards of safe and compassionate planned care to people that maximises their recovery and rehabilitation potential.
* Carry out programs of rehabilitation set by therapists and nursing team.
* Assist clinical staff carrying out clinical observations of patients on the inpatient unit.
* Support and motivate patients to participate in their rehabilitation through excellent communication skills.
* Have a desire to work in NHS, be motivated, flexible, with excellent communication and time management skills.
* Participate in training, education and clinical supervision to develop skills and knowledge and apply continuous reflection on own practice.
* Work as part of a team and alone in the community.
* The use of a vehicle to undertake any necessary travel in connection with the duties of the post is essential.

Job responsibilities

JOB SUMMARY

The post holder will:

* Under the supervision/direction of registered health care professionals/associate practitioner, within a delegated framework for agreed tasks, seek support as and when necessary, in the delivery of psychological and physical care to a defined group of people.
* As part of a healthcare team, provide the highest standards of safe and compassionate planned care to people that maximises their recovery and rehabilitation potential.
* Develop and maintain meaningful therapeutic relationships with people. Adopt a polite, positive and empathetic approach, communicating effectively and informatively.
* Demonstrate an understanding of the care and treatment programmes of a designated group of service users to enable you to instruct, advise and guide service users, family and carers. Make appropriate adjustments independently within your own level of competence and communicate changes to service users and registered health care professionals.
* Concentrate for periods of time as required by the care intervention/care plan, following observation policy, taking and recording physiological observations, and responding to issues of concern appropriately. May include areas of work requiring physical effort with regard to risk, health and safety.
* Exercise a degree of independence, initiative and judgement and consider different approaches when delivering tasks. Assess situations, identify and resolve potential problems within own levels of competence.
* Ensure the care experienced is positive and service user safety is maintained. Treat everyone with respect, dignity, courtesy and in accordance with the Trusts values.
* Contribute to the care planning process. Understand the principles underlying care planning, enabling the service user to reach an optimum level of functioning, independence and recovery.
* Implement care within well‑established policies, procedures, protocols, guidelines and established clinical care plans. Organise a range of activities to facilitate meaningful activities for individuals or small groups.
* Apply knowledge of longer term health conditions (eg, Diabetes, Obesity, smoking cessation) providing advice and support.
* Undertake the physical aspects of care (eg, moving and handling of service users). Use equipment (eg, hoists, wheelchairs) with appropriate instruction on their use.
* Assist in dealing with verbal/physical aggression using de‑escalation techniques; this may include restraint and/or seclusion as appropriate to the role.
* Attend to service users’ personal care needs as required.
* Participate in a flexible rostering arrangement providing 24‑hour/7 day care as appropriate to the role.
