Role

The Scottish Government promotes social prescribing as a way to reduce health inequalities. Social prescribing connects people attending GP or primary care services with local, often non-clinical, community resources.

Clackmannanshire and Stirling Health and Social Care Partnership (CSHSCP) has a Strategic Commissioning Plan focusing on prevention, early intervention, and reducing social isolation and loneliness. Investment in the Community Link Worker (CLW) programme aligns with this strategy, and we are seeking a new Community Link Worker to join the team in Clackmannanshire.

The Community Link Worker will work closely with practice staff to inform and connect service users to community resources, supported by CTSI/SVE, and will collaborate with local volunteer Community Health Champions.

The role involves helping people access local support from Third Sector organisations, aiding them in reconnecting and understanding how to improve and sustain their health and wellbeing.

* Hybrid: Stirling—based in GP surgeries, a city centre office, and from home, with some local travel.

We seek a motivated and compassionate individual with experience working with people with poor mental health, and understanding of personal outcomes, person-centred support, and trauma-informed practice.

The CLW team, established in 2022 and supported by the Clackmannanshire and Stirling Health and Social Care Partnership, has recently been awarded ‘Team of The Year’ at the VHS Community Link Worker Awards.

The CLW will be based in GP practices, helping people access local support from Third Sector organisations, and will be integrated into daily practice work, line managed through SVE, and collaborating with local volunteer Community Health Champions.

SVE is Stirling’s local third sector interface (TSI), aiming to build empowered communities through community groups, voluntary organisations, charities, and social enterprises. SVE offers services to enhance third sector capacity, focusing on excellence, resilience, social innovation, volunteering, community development, and involvement in planning and decision-making.

SVE is committed to inclusivity and professional development, offering benefits such as:

Our modern office is located near town and train station, with amenities including shops, restaurants, and green spaces.

Clackmannanshire Third Sector Interface (CTSI) provides support for charitable organisations, community groups, volunteering, and social enterprises in Clackmannanshire. It is part of the TSI Scotland Network, funded by the Scottish Government, with key functions including capacity building, advocacy, knowledge sharing, and partnership facilitation.


Membership & Governance

This role involves leading a small team, ensuring member compliance with business operations, governance, funding, staff management, data compliance, sustainability, and health and safety. The role includes providing advice, liaising with national bodies like OSCR and Social Enterprise Scotland, and supporting training and development. The post holder will work closely with the Chief Officer and other team leaders.


Family Support

The role will oversee the development of the Family Support Collaborative, linking around 30 third sector organisations to deliver a new care model with statutory partners.

The post includes managing a dedicated team, including a Family Support Collaborative Coordinator and a Community Connector, overseeing Family Support Hubs, data management, and producing a Children & Families Bulletin, as well as running quarterly forums.


Community Link Worker in Inverclyde

Funded by NHS Greater Glasgow and Clyde, these workers are based within GP practices and work across community, third sector, and public sectors to support well-being by strengthening community connections. They support individuals to identify personal outcomes, address barriers, and connect to local and national support services.

* Hybrid: Clarkston and Barrhead, with travel across East Renfrewshire required.

Voluntary Action East Renfrewshire SCIO (VAER) supports local communities.


Job Role: Youth Lead (Networks & Opportunities)

We seek an experienced, proactive, and organized individual passionate about working with young people to join our team. The role involves building capacity, facilitating collaboration, and ensuring inclusive volunteering opportunities for young people aged 12-25. The programme, funded by Young Start via the National Lottery Community Fund, includes developing youth-friendly volunteering, fostering collaborations, creating networking opportunities, and involving young people in programme design and delivery.
