Job overview

An exciting opportunity has arisen to become a Community Staff Nurse with the Amersham Adult Community Healthcare Team. The role involves providing nursing interventions to patients in their home environment. If you are interested in joining our friendly team, we would love to hear from you. You must be a car driver with access to a car for work.

In this role, you will become a competent Community Nurse. Our skills include IVs, catheter care, palliative care and end of life support, leg ulcer management, wound care, medication management, complex care management, prevention of admission work, and MDT working.

Our days are varied, with teamwork and compassion being at the heart of everything we do. Amersham ACHT aims to provide the highest standard of care for our patients. We will support you through your induction and skill development. If you have any questions, please call us or visit us.

Ideally, you should have a wide range of clinical expertise within the NHS and experience working in a multidisciplinary team. The post holder must demonstrate professional skills and competence in medicines management, care, and treatment delivery. You should be innovative and willing to develop your knowledge and skills.

The ACHT provides a 24/7 service.


Main duties of the job

* Assess the physical and psycho-social needs of patients and deliver therapeutic interventions based on evidence to improve health outcomes.
* Manage your own caseload, delivering and evaluating services for patients with various medical conditions.
* Plan interventions with the Case Manager and team members using the common assessment process.
* Participate in referral management to ensure patients are treated by the most appropriate team member/service.
* Contribute to the development of protocols and procedures for clinical pathways.
* Assist in developing case management protocols and guidelines within your professional practice. Participate in weekend/7-day rotas.

* Registered Nurse
* Recent CPD evidence


Desirable

* Experience in Community nursing (as a student or registered nurse)
