About the role - The Transitions Team is based at the London of Borough Barnet offices in Colindale and is managed alongside the Barnet Learning Disabilities Service within the Adult Social Care and Heath Directorate. The team consists of social workers, family support workers, Lead Practitioner and a team manager. The team works operationally across Barnet with all agencies involved in transition planning for young people moving into adult services who are eligible for care and support under the Care Act. The Transition Team will assess any young person between 17.5 to 25 years with a primary diagnosis of any form of disability, sensory impairment etc. - The workers in the team undertake the Adult Social Care and carer's assessment under the Care Act 2014 to determine eligibility for ASC and the support needed. Type of Work This flexible 36-hour weekly role permits a combination of office and remote work. The team are in the office at least once a week on Tuesdays and other days as needed. Parking/ Transport - Barnet have parking available on pre-booking basis, with disabled parking also available. Colindale station is located 5/10 minute walk from our offices. Equipment - Laptops and accessories will be provided, including mobile phones if needed, and office equipment for hybrid workers. You will also receive an IT induction to help you navigate our laptops and systems. Colindale Offices - The borough of Barnet has many restaurants/cafes and outlets to enjoy for all agency workers. Our modern Colindale office has a coffee shop, and rooftop decking including quiet work spaces and private meeting rooms. There is also free coffee and tea for onsite workers. Office-based work is also an option, with free parking available at our Colindale office. - Working alongside the manger you will mentor, supervise and support staff along with being a visible and support leader who works in creative ways in a confident and calm manner be comfortable to provide challenge when required. - Being part of a supportive team you will also help individuals to realise their life goals, placing wellbeing at the core of our efforts. - You will hold a small case load of complex cases including safeguarding and sometimes court work - Support practitioners to enable young people and their families to have a smooth transition into adult services. This will include working with colleagues in Children's Services, Education, and health services. - Provide professional support via mentoring and supervision to staff within the team ensuring professional standards and best practice are always upheld. - Work alongside and when required deputising for the team manager to prompt an environment of a supportive team enabling them and yourself to work with individuals to promote and support independence choice and control with wellbeing at the heart of everything we do. - Conduct individual complex assessments and safeguarding enquiries using the appropriate tools and techniques in order to establish clear, relevant, proportionate, and appropriate options for the individual to meet their identified outcomes. - Work in partnership with adults, their families, carers and advocates, colleagues, external organisations to provide appropriate, professional social work support for vulnerable older people, adults with physical and sensory impairment or learning disability and carers - Conduct Mental Capacity Act assessments in line with statutory guidance and ensuring adults are provided with the opportunity to demonstrate capacity wherever possible. - Ensuring that the responsibility of safeguarding adults at risk is a fundamental element of this job and that the principles are embedded in all procedures, practices, professional advice, decisions and support.
