Memory Assessment Clinician

An exciting opportunity to join the Community mental health – Older adult service, an expanding team working with patients with dementia and functional disorders. The role of the Memory assessment clinician is open to CPNs and Occupational Therapists interested in developing their skills in Memory assessment work. The role will involve working predominantly with patients presenting with memory difficulties, completing assessments of cognition, and supporting the diagnostic process. As the team grows the scope may include work with patients presenting with functional illness.
Key responsibilities
* Conduct memory assessments in clinic or patient's home environment, depending on patient need.
* Provide timely diagnosis in close joint working with Consultant psychiatrists.
* Administer pharmacological treatment guidance from Consultant psychiatrists as appropriate for the profession.
* Offer post‑diagnosis support and signposting.
* Monitor and review patients started on treatment.
* Contribute to duty system.
* Assess risks at home and support mitigation using MDT approach.

Person specification
Qualifications
* Relevant professional qualification e.g. RMN, DipCOT.
* Current unrestricted registration with HCPC/NMC.
Experience
* Substantial experience in a Community Mental Health care setting.
* Developed specialist knowledge within a similar environment over a considerable period.
* Significant experience in undertaking assessment, formulation, risk assessment and positive risk management.
* Proven experience of people management.
Other requirements
* Must have full driving licence and access to a motor vehicle for business use.
