AN-AAC COORDINATOR | | Wyee Permanent Part Time opportunity of 21 hours per week at our Bethshan Gardens Facility in Wyee About the role To provide coordination of the AN-ACC process, to lead, conduct pre-admission and complete a suite of assessments supporting the AN-ACC Assessment tools. To ensure optimal classification and funding for BaptistCare. The position will ensure that BaptistCare pre-admission processes are followed. To coordinate and facilitate ongoing review and request for reassessment when resident’s condition has changed significantly. To complete any supplemental funding claims to both, maximize and risk manage the home’s government funding, in consultation with Funding Consultant and Client Liaison Team to achieve and maintain budget funding targets. Compile and follow the AN-ACC Checklist as per Framework and Reference Manua About You Must be a Registered Nurse. Demonstrated capacity to provide leadership in articulating BaptistCare’s Purpose and Values in management practices and commitment to actively promote the Purpose and Values. Knowledge and experience in Aged Care. Demonstrated knowledge and understanding of the Australian National Aged Care Classification (AN-ACC).
