Community neuro caseload across home and community settings: Stroke (ESD), ABI, MS, Parkinson's, MND.
What you'll do
* Functional, goal‐focused neuro rehab in the home
* Cognitive/perceptual assessment, upper‐limb rehab, fatigue/tone management
* Reablement, equipment/adaptations, seating/positioning
* Home visits, risk assessments, ESD and safe discharge planning
* MDT working and clear documentation (e.g., COPM, GAS, MoCA, ARAT)
What we're looking for
* HCPC‐registered OT
* Recent Band 6 community neuro experience (NHS preferred)
* Confident with domiciliary assessments and risk management
* UK RTW + enhanced DBS
* Car driver
Nice to have: ESD stroke experience, splinting/positioning, Bobath handling.
