We have an exciting opportunity for enthusiastic and versatile individuals to join the Trust as a Patient Pathway Coordinator and administrator in the Upper GI Team, Gastroenterology department.
Patient Pathway Coordinator deals with 2ww Cancer patients pathway, ensuring that all data is updated in a timely manner. Having a hard-working and forward-thinking philosophy with patient care at the focus of what we do.
Apprenticeship Eligibility Criteria:
All applicants must be:
* Aged 16+
* Resident in the UK/EEA for the last three consecutive years
* Must not have a qualification in a similar subject at the same level or above
* Must not be in any form of formal government-funded education upon the start of their apprenticeship
Qualifications and Skills Required:
* Good general standard of education
* 4 GCSE's at Grade D/3 or above (or equivalent) subject to the job specification
* Good standard of literacy and numeracy
* Good IT skills
Full time: 37.5 hours per week
Main duties of the job
The post holder will be a member of a small admin team and will provide an essential first point of contact between the hospital, patients and their relatives, and all other healthcare professionals. They will deal efficiently and effectively with direct and indirect patient contact.
They will have responsibility for the administration of clinics. They will monitor patient waiting lists according to the Patient Focus Booking policy and will provide professional reception duties. They will need to work flexibly to cover different shifts which will include early, evening and weekend shifts. The post holder will be required to track and collect patient notes and be able to work well autonomously and as part of a team.
They will occasionally take minutes and attend monthly team meetings.
General working days to include Monday to Friday. There is a requirement to work early and late evening shifts as part of an initiative in Medical Outpatients to extend clinic hours: clinic times are between 7.30am and 9pm. Reception team are required to cover between these times.
We are looking for cover 5 days per week.

Job responsibilities
* Actively track patients urgently referred to the Trust from General Practitioners (GP) who are suspected at referral of having Cancer. Ensure that these patients are listed on the Trust Cancer IT system.
* Ensure that appropriate requests for investigations are initiated following the clinicians instructions and that appointments are given.
* To adhere to the Trusts General Principles and Guidelines on Patient Access in relation to the management of waiting lists.
* Ensure that any newly diagnosed Cancer patients are identified and added to the Trust Cancer IT system. Ensure that the patient is known to the clinical team and that appropriate action is taken to manage the patient on the agreed pathway.
* Highlight any potential delays or breaches of the patient pathway according to the Trust escalation policy.
* Where it is not possible to manage patients through the pathway by the target dates, to ensure completion of breach reports. To be pro-active in suggesting changes to the pathway and facilitating change.
* Provide accurate and confidential information to assist with continued data entry, strictly adherent to Caldicott and Data Protection Act and retrieval of patient information via the PAS, RIS, ICE and other departmental computer software programmes.
* Assist patients and their carers by arranging translation/interpretation facilities via INTRAN.
