About the Role: We are seeking a highly motivated and compassionate Social Worker to provide specialist support and advocacy for adults with learning disabilities and autism. In this vital role, you will be instrumental in: Conducting comprehensive assessments of needs, strengths, and risks for individuals with learning disabilities and/or autism, ensuring a person-centred approach. Developing and reviewing personalised care and support plans that promote independence, well-being, and community inclusion. Working collaboratively with individuals, their families, carers, and a range of multi-agency partners (e.g., health professionals, housing, education, employment services). Safeguarding vulnerable adults and responding effectively to safeguarding concerns, ensuring robust risk management. Navigating complex transitions (e.g., from children services, into adulthood, or changes in living arrangements). Providing expert advice and guidance on relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005. Advocating for the rights and choices of individuals with learning disabilities and autism, empowering them to live fulfilling lives. About You: You will be a Qualified Social Worker with: Substantial experience working with adults with learning disabilities and/or autism. A deep understanding of the challenges and complexities faced by this client group. Expert knowledge of relevant legislation, policy, and best practice in adult social care, particularly concerning learning disabilities and autism. Excellent assessment, analytical, and communication skills, with the ability to engage effectively with diverse individuals. A proactive, empathetic, and resilience approach to your work.
