Who You Are A dedicated and compassionate professional with a Diploma in Social Work (DipSW) or Certificate of Qualification in Social Work (CQSW), and HCPC registration. You possess substantial experience and knowledge of mental health work within a statutory framework. You have a strong background in participating in multidisciplinary teams and have experience in care management. Your excellent written and verbal communication skills enable you to effectively engage with service users, carers, and other professionals. What the Job Involves The role involves providing assessment, care management, and social work services to adults with mental health problems, and their carers, in a community setting. As part of a multidisciplinary team, you will establish service eligibility, manage and review care plans, and coordinate care according to the Council’s policies and financial regulations. You will also handle safeguarding adult enquiries, work within duty rota systems, and undertake training such as becoming an Approved Mental Health Professional (AMHP) and Best Interest Assessor. Maintaining compliance with local and professional documentation standards and keeping updated on mental health legislation and policies are central to the role.
