Care Planner - Prestige Nursing & Care Blackpool
Full-time | Permanent | Monday to Friday (plus on-call rota)
Full UK Driving Licence and own vehicle required
What You'll Be Doing
* Creating and managing staff rotas using Webroster
* Allocating, processing, and mobilising new care package referrals
* Conducting care assessments and manual handling reviews in client homes
* Ensuring staff training, compliance and documentation is up to date
* Supporting recruitment and onboarding of new care staff
* Liaising with families, case managers, commissioners and other professionals
* Participating in the on-call rota alongside the wider office team
Job Profile
The Care Planner performs a critical role within a Prestige Nursing branch by helping to organise and monitor the homecare services provided by the branch, as well as supporting the branch manager in the day-to-day administrative tasks of running a busy office. It would suit an experienced and IT-literate care worker with good administrative skills and telephone manner that is also able to perform risk assessments in line with their duties.
What You'll Need
* Experience in care coordination or a strong understanding of home care
* Excellent administrative, communication and IT skills
* Calm and solution-focused approach under pressure
* Knowledge of CQC compliance, safeguarding and risk management
* A full UK driving licence and access to your own vehicle

* Contracted Hours: 37.5 hours per week (Monday to Friday, with a 30-minute unpaid lunch)
* Place of Work: Prestige Nursing Blackpool, 29 Nutter Road, Thornton-Cleveleys, FY5 1BQ - this is not a remote role
* Base Salary: £25,915 per annum
* On-Call Supplement: £2,925 per annum (based on one weekday per week and one weekend in four)
* Total Salary Package: £28,840 per annum
* Additional Hours: Any care or work undertaken outside contracted responsibilities paid at £13.29/hour
* Review: Full package reviewed on successful completion of six-month probation
