Tapping House is a leading provider of palliative care services in Norfolk. We are an independent charity delivering innovative and specialist rehabilitative therapy, end of life care and bereavement services.
Rehabilitative Palliative Care integrates rehabilitation, enablement, self management and self-care into the holistic model of palliative care. The specialist physiotherapy post has been developed to promote and embed this patient-centred approach with patients attending Therapy and those who receive end of life care at home. The purpose is to support people to live fully until they die. The Tulip Centre will see people at all stages of their health journey, from diagnosis where there can be uncertainty over prognosis, through to deteriorating advanced disease. Most interventions will occur in the department, but there is a recognised need to complement existing community therapy services and the hospices inpatient unit, in providing support, particularly with breathlessness and anxiety management. Both the Hospice at Home service provides care and support for people recognised to be in their final weeks of life. The physiotherapist will provide interventions at home to support, as required, those people with difficult symptoms such as breathlessness and distress, and in supporting functional difficulties.

Responsibilities
* Clinical: be professionally and legally responsible for all aspects of your work, including the management of patients in your care and the supervision of interventions provided by Rehabilitation Assistants to these patients.
* As part of the therapy team, manage the therapy caseload with key worker responsibility for patients undergoing a predominantly physiotherapy focused plan of care.
* Use evidence-based and patient-centred principles in planning, implementation and evaluation of physiotherapy interventions.
* Maintain patient-centred goals and engage in treatment and support programmes with emphasis on adaptation to illness, changing function and developing self-management strategies.
* Communicate sensitively with patients, carers, families and staff using advanced communication skills.
* Attend multidisciplinary meetings and contribute to the multidisciplinary approach to care.
* Identify needs within a holistic framework, including physical, psychological, social and spiritual aspects, fostering an interdisciplinary approach to rehabilitation.
* Lead self-management groups addressing breathlessness, fatigue management and exercise; work with the multidisciplinary team to implement therapeutic programmes for individuals and groups.
* Maintain clear referral processes for external and internal services and support smooth transitions between services.
* Maintain accurate and contemporaneous clinical records (SystmOne).
* Liaise with carers and external agencies regarding therapeutic interventions; contribute to discharge planning from the Tulip Centre and other services when appropriate.
* Delegate work to and provide support for Therapy Volunteers.
* Support patients under the Hospice at Home team and in the inpatient unit with physiotherapy input, including advice on managing respiratory deterioration, mobility and pain.
* Support future hospice developments requiring physiotherapy input.
Education, Development & Management
* Manage, supervise and appraise junior staff, students and volunteers.
* Contribute to safe and competent use of equipment/aids and ensure staff competency in their use.
* Instruct others in moving and handling techniques as required by individuals and environments.
* Engage in ongoing professional development through development reviews, supervision and planned education programmes.
* Lead quality improvement projects and clinical audits; contribute to service objectives, protocols and policies.
* Involve in orientation, development and support of other staff and students; mentor and supervise less experienced staff.
* Foster continuous learning and evaluation within the team and care volunteers development.

Education & Qualifications
* Degree or diploma in Physiotherapy
* HCPC Registration
* Evidence of relevant post-graduate courses/CPD
* Desirable: Further qualification in end-of-life care or equivalent
* Advanced communication skills training
* Membership of Chartered Society of Physiotherapy & Association of Chartered Physiotherapists in Oncology and Palliative Care
Experience
* Essential: A minimum of 24 months experience as a junior physiotherapist; recent post registration experience in a relevant setting (palliative care, acute medicine, neurology, oncology or community).
* Experience of working with people nearing the end of life and using outcome measures to enhance service delivery.
* Desirable: Evidence of staff supervision, experience involving leave planning, rotas and minor employee relations issues.
* Experience of supervision and appraisal of junior therapists and/or students; previous experience working with volunteers.
* Experience of working with people nearing the end of life.
