Whats the role all about?
As a Care Navigator, you will be at the heart of our multidisciplinary team, providing essential support to vulnerable and frail individuals in our community. Your responsibilities will include:
* Being a first point of contact for patients and clinicians via phone
* Coordinating appointments and updating clinical rotas
* Liaising with multiple health and care services
* Managing and processing essential documents and data
* Supporting the smooth operation of the Frailty Hub

Main duties of the job
* Act as a point of contact for individuals, families, and other healthcare professionals
* Communicate with patients over the phone professionally, creating a positive patient experience
* Accurately record patient details on SystmOne clinical system with good attention to detail
* Handle patient information confidentially
* Work both independently and as part of a team in a busy office environment
* Book appointments ensuring patients are fully informed and follow-up as necessary
* Retrieve information efficiently via all clinical systems following Nimbuscare policies
* Communicate effectively with the team to support informed decision-making regarding patient care
* Manage administrative requests from colleagues and other team members
* Take full responsibility for assigned administrative areas of work
* Perform all duties effectively and efficiently in line with guidance and protocols
* Work flexibly to meet the needs of Frailty Hub/YICT
* Adhere to all Nimbuscare policies & procedures

Qualifications

Salary: £25,065 to £26,734 per year (Full-time Equivalent)
