We are expanding our Care Navigator team at Nimbuscare and looking for someone motivated, compassionate, and ready to take on a rewarding challenge.


Whats the role all about?

As a Care Navigator, you will be at the heart of our multidisciplinary team, providing essential support to vulnerable and frail individuals in our community. Your responsibilities will include:

* Being a first point of contact for patients and clinicians via phone
* Coordinating appointments and updating clinical rotas
* Liaising with multiple health and care services
* Managing and processing essential documents and data
* Supporting the smooth operation of the Frailty Hub

Main duties of the job

* Act as a point of contact for individuals, families, and other healthcare professionals
* Communicate with patients over the phone professionally, creating a positive patient experience
* Accurately record patient details on SystmOne clinical system with good attention to detail
* Handle patient information confidentially
* Work both independently and as part of a team in a busy office environment
* Book appointments ensuring patients are fully informed and follow-up as necessary
* Retrieve information efficiently via all clinical systems following Nimbuscare policies
* Communicate effectively with the team to support informed decision-making regarding patient care
* Manage administrative requests from colleagues and other team members
* Take full responsibility for assigned administrative areas of work
* Perform all duties effectively and efficiently in line with guidance and protocols
* Work flexibly to meet the needs of Frailty Hub/YICT
* Adhere to all Nimbuscare policies & procedures

Job responsibilities

* Liaise with patients and external agencies to resolve queries
* Act as a point of contact for individuals, families, and healthcare professionals
* Communicate professionally with patients over the phone, ensuring a positive experience
* Record patient details accurately on SystmOne
* Handle patient inquiries responsibly and follow through to resolution
* Maintain confidentiality of patient information
* Work independently and as part of a team in a busy environment
* Book appointments and ensure patients are well-informed
* Retrieve information accurately following policies
* Communicate effectively within the team to support patient care decisions
* Manage administrative requests appropriately
* Take responsibility for assigned administrative tasks
* Maintain up-to-date statutory and mandatory training
* Work flexibly to meet the needs of the service
* Follow all policies & procedures
* Perform other duties as required for the efficient operation of the service

* Ability to travel across various sites when required


Experience

* Proficient in Microsoft Applications
* Excellent communication skills at all levels
* Ability to work under pressure and meet deadlines
* Teamwork and initiative
* Self-motivated with a positive attitude
* Empathy and good listening skills
* Hardworking and adaptable
* Commitment to professional development


Qualifications

* Minimum Level 2 Maths and English or equivalent (GCSE Grade C or 4+)
* High standards of accuracy and attention to detail
* Good numeracy, organizational, and administrative skills
* Experience in NHS or general practice, especially with SystmOne


Disclosure and Barring Service Check

This role requires a DBS check due to the nature of the work involving vulnerable populations.

Salary: £25,065 to £26,734 per year (Full-time Equivalent)
