The Opportunity
We’re seeking a dynamic, experienced healthcare professional to join us as an Associate Case Manager. This is a flexible, self-employed role ideal for someone who thrives on autonomy, clinical challenge, and meaningful client relationships.
* Deliver proactive, client-centred case management for individuals with complex injuries
* Coordinate rehabilitation pathways and support return-to-function goals
* Liaise with multidisciplinary teams to ensure seamless care
* Advocate for clients’ needs with empathy and strategic insight
Based in Middlesbrough, we serve clients across the North East and North Yorkshire.
What You’ll Bring
We’re looking for someone with:
* A relevant professional qualification (Social Worker, Occupational Therapist, Physiotherapist, or Nurse)
* At least 3 years’ post-qualification experience, including work with brain injury, spinal cord injury, and/or amputation
* Minimum 2 years’ experience in case management
* Registration with your professional body
* Strong knowledge of rehabilitation pathways
* Self-employed status with professional indemnity insurance
* A car and business insurance for travel
Pay & Checks
* Competitive self-employed rates (negotiable based on experience)
* Enhanced DBS required (we can arrange this if needed)
* Proof of right to work in the UK, professional registration, references, and driving licence (minimum 12 months driving experience) required
