What you will do
* Undertake person-centred assessments and empower individuals to make informed choices.
* Collaborate with people who have complex needs to create support plans that maximise abilities and independence.
* Contribute to the development and continuous improvement of Adult and Integrated Care Services, ensuring services meet local and national standards.
* Provide person-centred assessments and develop care plans to achieve identified outcomes.
* Liaise effectively with agencies and other professionals to meet identified needs.
* Promote independence and wellbeing, ensuring care services are focused and person-centred.
* Support planning using strength-based approaches, maximising individuals’ strengths and community resources.
* Manage a diverse and challenging caseload, including safeguarding duties and court appearances.
* Maintain high data quality in all recorded information, especially in safeguarding and care reports.
