Macmillan Cancer Care Navigator - Fixed Term

We are seeking exceptional personnel to join us in delivering best practice personalised care and support interventions to cancer patients and their families as part of our Living with and Beyond Cancer Program here at Sandwell and West Birmingham Hospital NHS Trust.
Main duties of the job
Working closely with Cancer Services and our Cancer Nurse Specialist Team, you will be a source of contact for cancer patients and their families, helping them to navigate through their cancer pathway from diagnosis and into follow‑up. You will provide reassurance, telephone patient triage and support, patient‑related information giving and signposting. You will also support the CNS teams in the smooth running of living with and beyond cancer services, including undertaking administrative duties related to nurse‑led clinics, MDT outcomes and tracking patients on remote surveillance. Additionally, you will deliver Holistic Needs Assessments and Care Planning telephone clinics, supporting patients in resolving non‑complex needs and concerns, and manage patients who are on patient‑stratified follow‑up through remote monitoring.
Job responsibilities
The post holder must demonstrate enthusiasm, commitment and a drive to work with cancer patients and carers. The ideal candidate will possess expertise, skills and knowledge of cancer, and the ability to learn and develop their knowledge and abilities to carry out Holistic Needs Assessments throughout the patient journey and develop care plans. They will signpost patients to services that support self‑management. Excellent communication skills are required, confident in dealing with patients both on the phone and face to face, while respecting the limits of expertise and role. Demonstrable evidence of competent IT and office administration skills, and experience of information patient systems to maintain accurate and timely electronic patient records, is essential. The role offers a great opportunity to be vital to the personalised care and support agenda. The role is for 12 months.
Qualifications
* GCSE English language and Mathematics (Grades A‑C) or equivalent
* Recognised ECDL or equivalent or extensive experience using Microsoft Office package
* Communication skills training (e.g., Thyme and Sage)

* Experience of multi‑professional working
* Experience in the use of data management
* Experience of coaching/teaching patients and carers
* Experience in Cancer Care
* Understanding of Macmillan Cancer Support and its role across the UK
* Awareness of Improving Outcomes: A Strategy for Cancer and other national cancer policies
