We are seeking a dedicated and compassionate Practice Nurseto join our team. In this role, you will provide highquality nursing care toour patients, contribute to ongoing health promotion, supporting our cliniciansand working collaboratively with our administrative staff.

Main duties of the job
The successful candidate will be responsible for deliveringhigh quality nursing care to our patients, including undertaking clinicalassessments, managing long term conditions, administering vaccinations, andperforming procedures such as ECGs and venepuncture.
They will provide health promotion and patient education,support preventative care initiatives, and contribute to the development andmonitoring of individual care plans.
The successful applicant will also be expected to maintainaccurate clinical records, adhere to infection control and safety standards,and work collaboratively with the wider multidisciplinary team to ensure safe,effective and patient centred care.

Job responsibilities
The successful candidate will ideally demonstrate thefollowing skills and experience
Competence in administering vaccinations, including routineadult and seasonal immunisations.
Experience delivering childhood immunisations in line withnational vaccination schedules.
Proficiency in cervical screening, including managing nonresponder recalls and ensuring timely follow up.
Experience in the management of long term conditions as partof ongoing patient care.
Ability to undertake hypertension reviews and supportpatients through monitoring and lifestyle guidance.
Experience providing anticoagulation services, including themonitoring and management of patients on Warfarin and DOACs.
Training may be provided as required.
Strong clinical judgement and the ability to workautonomously within scope of practice.
Excellent communication skills with a compassionate, patientcentred approach.
Good organisational skills with attention to detail inmaintaining accurate clinical records.
Ability to work effectively as part of a multidisciplinaryteam in a busy practice environment.
Welsh language skills would be desirable but are notessential for this role.

Qualifications
* Registered General Nurse (RN) qualification with valid NMC registration to practice.
Experience
* Strong interpersonal and communication skills.
* Proven ability to work independently and as part of a multidisciplinary team.
* Excellent organisational and record-keeping skills
* Experience of working in primary care
